Consent To Treat Form:
Please read the ST Osteopathy Treatment Consent Form which simply means you have read and understood what the appointment will involve and give your permission for treatment.
You can either print this form off and sign before your appointment and bring it with you to the appointment, or we can provide you with a Treatment Consent Form to sign on your first appointment. Download Consent To Treat Form
Parking In Swaffham:
- The best place to park is in Theatre Street car park (Theatre Street, Swaffham PE37 7HF) or where available further along Whitsands Road
- Please remember the Swaffham Market is on Saturdays & it is recommended to park for free in Theatre Street Parking (PE37 7HF).
- Please do not park directly opposite The Philosophy Rooms as it is private parking and tow trucks and clamping is a regular occurrence
